Patch Trend plots one point per patch for the champions you select, using the metric you choose — Presence, Ban%, or Pick%. Each point is computed only from the games played on that patch, so the line shows how the champion's standing moved as balance changes rolled out.
Professional metas rarely stand still. A champion's line typically rises after direct buffs, after an item or system change that suits it, or after a strong team shows what it can do on stage and others copy the idea. Falling lines follow nerfs — or simply the discovery of a reliable answer, which can push a champion out of the meta without any patch note mentioning it.
A good starting point is the biggest movers shown on the home page: overlay a few of them (up to five) and you can see, for example, two junglers trading places as the pool shifts, or a mid-lane pick collapsing the patch after its core item was changed.
Two caveats: the newest patch always has the fewest games, so its point is the noisiest, and regions adopt patches on different schedules — an international event or a split break can leave a region with very few games on a given patch.
